Join/add/change:

• Get an insurance quote - use this to obtain a quote for you and your spouse/partner for life, disability or medical insurance through SuperLife
• Product disclosure statement & membership form: individual - to setup a superannuation scheme as an individual member
• Product disclosure statement & spouse membership form – insurance & savings - to set up a superannuation scheme for your spouse/partner for savings and/or insurance
• Life and/or disability insurance - to add or change your life and/or disability income protection insurance benefit in your existing SuperLife workplace savings scheme membership. To apply for insurances if you are not a SuperLife workplace savings scheme member, you will need to fill out a membership form for SuperLife Invest here.
• Medical insurance - to add or change your UniMed insurance cover benefit or to update your dependants to your existing membership

Update:

• Update personal details and/or beneficiary details - use this form to update:
  • Personal details - such as address, names and contact details
  • Beneficiary details - update the beneficiaries who will receive the benefits that are payable if you die

Inform:

• Non-smoker declaration - use this form to advise SuperLife if you were a smoker and have become a non-smoker
• Update smoker status - use this form to advise SuperLife of your current smoker status for the purpose of calculating any life insurance premiums
• Stop insurance cover - use this form to stop your voluntary life and/or disability and/or medical insurance benefits
