• SuperLife Invest product disclosure statement and application form - to join SuperLife invest as an individual or through an entity

Add/change:

• Change savings contributions - use this form to change your current monthly savings amount if you are an Individual Member
• Change investment strategy - use this form to change your investment strategy for your existing and/or your future savings
• Switch investments - use this form to switch some or all of your existing savings between different investment pools

Update:

• Update beneficiary details - use this form to update the beneficiaries who will receive the benefits that are payable if you die
